This section topic describes the seven default survey Question Types provided by Aptify. Administrators familiar with XSL (Extensible Style Language) can create additional Question Types. See "About the Question Types Form" on page 36 for  for additional information.

This Question Type specifies a multiple-choice question with one answer. It creates a drop-dropdown down menu that contains the possible answers, as shown below.

You can use this Question Type to provide instructions or messages to the Web web user. The figure below shows how the sample Welcome question, which uses the Header type, is presented to web users. You do not need to configure Possible Answers for a Question that uses the Header type.

This Question Type creates a matrix that includes the following column headings: Poor, Fair, and Excellent. Each Possible Answer is a separate row in the question. A web user selects one column for each row.

For example, in the figure below, the Possible Answers are Content, Layout, and Length. A web user assigns a rating to each of the Possible Answers, choosing between radio buttons for Poor (1), Fair (2), and Excellent (3).

Short Answer

This Question Type inserts a text field so a Web user can personalize his/her response to the survey question. For this Question Type, you should configure one Possible Answer for each field you want to create. Each answer corresponds to the label that will appear to the left of the text field.

For example, in the figure below, the Possible Answer to the Question is "E-mail"; this text appears to the left of the text field on the survey screen.
