Answer Sheets records maintain information on how each student answered questions on a particular exam.

Answer Sheets Form

Top Panel

Student (Required)

The name of the student who is taking the exam. This field links to the Persons service.

Type (Required)

The Answer Sheet's type:

  • Regular: Regular is the default. Select Regular if the student is taking a course that is normally required. If the exam is scored with the Scantron wizard, the system creates Regular answer sheets.
  • Substitute: Select Substitute if the student is taking an alternative course.

If you select the Regular exam type, populate the Exam and Class fields. If you select the Substitute exam type, enter the Course and complete the Substitute Course tab.

Exam

For Regular answer sheets, enter the name of the exam that the student is taking. Refer to Exams Form for information on the About the Exams Form.

Class

For Regular answer sheets, enter the name of the class for which the student is taking the exam. Refer to About the Classes Form for information on the Classes form.

Course

For Substitute answer sheets, enter the name of the usual course for which the student is fulfilling requirements. This is not the name of the substitute course that the student is taking. Because the student is taking a substitute course, this field contains the name of the equivalent, regular course.

Status (Required)

Enter the student status related to this exam. Select from:

  • Registered: The student has not yet taken the exam. Registered is the default status.
  • Passed: The student has taken the exam and has earned a passing score.
  • Failed:  The student has taken the exam and has not earned a passing score.

Hold Code

This is a legacy field that exists for compatibility with previous versions of the Aptify Education Management. Do not use this field.

Date Recorded (Required)

Enter the date that this student's answers are recorded for this exam. The current date is the default.

Serial # (Required)

Enter the exam serial number.

Point Score

The student's point score on the exam. After the Scantron wizard grades the Scantron answer sheet, the student's point score on this exam displays automatically.

Percent Score

The student's percentage score on the exam. After the Scantron wizard grades the Scantron answer sheet, the student's percent score on this exam displays automatically.

Answers Tab

The Answers Tab lists answers for the exam. The Scantron wizard scores student exams based on the input file and automatically updates the Answers tab on each student's Answer Sheets record. The wizard converts student answers on Scantron sheets to Answers records on the Answers tab. To manually add a new answer to the list, right-click in the gray area and select New. An Answers record displays. Refer to Answer Sheet Answers Form for information on the About the Answers Form.

Attachments Tab

Attach any documents relevant to this student's answer sheet on the Attachments tab.

Comments Tab

Enter any comments relevant to this student's answer sheet on the Comments tab.

Substitute Course Tab

If you specified the Substitute answer sheet type, complete the fields on this tab.

School

This field is available if you select Substitute in the Type field. Use the drop-down list to select the name of the school that offers the usual course for which the student is fulfilling requirements. 

The list contains schools listed on the Schools tab of the relevant Courses record.

This is not the name of the school where the student is taking the substitute course. 

Course Name

This field displays if you select Substitute in the Type field. Use the drop-down list to select the name of the substitute course that the student is taking. The list contains courses listed on the Course School Substitutes tab of the relevant Schools record. See Course School Substitutes Tab for more information on substitute courses.
